Welcome to the Geriatrics Interdisciplinary Teams in Practice (GIT-P) website! With support from the John A. Hartford Foundation, the GIT-P initiative was introduced to improve the quality of health care for older adults through supporting the development and careful testing of models of interdisciplinary team care. Through this program, four projects have been developed and are now available for adoption. While all four projects address the issues related to the use of interdisciplinary teams in geriatric care, each project differs in its approach and will be useful in different types of settings. Please use the table below to determine the needs of your organization and which projects will address those needs. |
